By filling out the signup form, you acknowledge that you have read and agree with the terms and conditions associated with this affiliate program.

By filling out the signup form, you will automatically become an affiliate and are bound by the terms of this agreement. 

Your affiliate application and status in the program may be suspended or terminated for any reason at any time. 

You are granted permission to use the marketing material provided to promote Waivo™.

Waivo™ in good faith agrees to pay a marketing fee of $25.00 for every qualified lead that schedules a consultation with a Waivo Account Executive.  Waivo™ in no way is associating the marketing fee with whether or not the lead makes a purchase.  This is simply a marketing fee.  It will be paid quarterly following the inception date of the enrollment of this affiliate program.
